Thinking Like a Clinician: Making Your Learning System Last All Year (Episode 4 of the Clinical Learning System Series)

And perhaps the biggest question of all… How do you know if you’re actually becoming a better clinician?

I think one of the most encouraging things about third year is that improvement often happens so gradually you don’t notice it. Then one day a patient begins telling their story… And before they’ve finished speaking… You find yourself thinking, “I’ve seen this before.” That’s one of the most satisfying moments in medical education. Not because you memorized another fact. Because you’ve begun recognizing patterns. I think that’s reassuring because many students feel like they’re not improving quickly enough. They compare themselves to residents… Or attendings… And forget those physicians have seen thousands of patients.

That’s an important perspective. You aren’t trying to think like someone who’s practiced medicine for twenty years. You’re trying to think a little better than you did yesterday. Progress during third year isn’t measured by perfection. It’s measured by pattern recognition. If today’s patient reminds you of someone you cared for three weeks ago… That’s progress. If your differential diagnosis becomes a little broader… That’s progress. If you’re asking better questions during the history… That’s progress. Clinical reasoning grows one patient at a time.

So how should students measure themselves? Because I think many people naturally default to scores.

Scores matter. COMATs and Shelf exams matter. COMLEX Level 2 and USMLE Step 2 matter. But they’re not the only scoreboards. I encourage students to ask themselves questions like… Did I recognize a pattern more quickly today? Did I understand why my attending made a different decision than I would have? Did I make fewer reasoning errors this week than last week? Did I improve one recurring weakness? Those questions measure the development of clinical reasoning. The exam scores usually follow.

I really like that because it shifts the emphasis from proving yourself… …to improving yourself.

That's right! Medical school sometimes convinces us that every question is a judgment. I’d rather students think of every patient and every question as feedback. Feedback tells you where to grow. That’s incredibly valuable.

One thing I struggled with during rotations was feeling uncomfortable when I didn’t know the answer immediately.

And welcome to medicine. None of us enjoys uncertainty. But uncertainty is one of the greatest teachers you’ll ever have. When you immediately know the answer… You rarely learn very much. When you’re uncertain… Your brain becomes curious. Curiosity creates attention. Attention strengthens memory. So instead of becoming frustrated when you don’t know something… Try saying, “Good. I just found today’s opportunity to learn.” That’s a very different mindset.

Another trap I see is students trying to create the perfect study system.

I see that all the time. They spend hours choosing apps. Reorganizing notes. Watching productivity videos. Color-coding everything. Those activities can feel productive… But they aren’t the same as learning. A good learning system isn’t the fanciest one. It’s the one you’ll still be using six months from now. Consistency almost always beats complexity.

I love the idea that patients become our teachers.

I do too. Think about what happens over an entire clinical year. One week you’re learning heart failure. The next week you’re seeing diabetes. Then pneumonia. Then inflammatory bowel disease. Then cirrhosis. Eventually your patients begin building your curriculum for you. Your job isn’t to memorize every disease after seeing it once. Your job is to recognize it a little more confidently the next time.

Where do Pattern Cards and Miss Logs fit once students have dozens of them?

Keep them alive. Every week spend a few minutes reviewing older Pattern Cards. Look through your Miss Log. Ask yourself, “Which weakness has improved?” “Which one keeps returning?” You’ll notice something encouraging. Problems that once felt impossible eventually become routine. And when that happens… Create room to learn the next pattern.

One thing I’ve heard you recommend before is teaching. Why is that so powerful?
Dr. Randy Clinch
Because teaching exposes the gaps in our own understanding. Try explaining today’s patient to a classmate. Or to your spouse. Or even to yourself while driving home. If you can’t explain something clearly… You probably don’t understand it as well as you think. Teaching is one of the best self-assessment tools we have.

Let’s imagine someone’s listening to this on the first day of third year. Where do you hope they’ll be a year from now?
Dr. Randy Clinch
I hope they won’t just have higher board scores. I hope they’ll walk into an exam room with greater confidence. I hope they’ll recognize common diseases more quickly. I hope they’ll ask better questions. Develop broader differential diagnoses. Communicate more effectively with patients. And maybe most importantly… Continue being curious. Because curiosity is one of the defining characteristics of outstanding physicians.

I do. Tomorrow… Choose one patient. Learn one clinical pattern. Build one illness script. Review one Pattern Card. Strengthen one weakness. Prepare for tomorrow’s patients. Then repeat.
